Accreditation 2025
To streamline submissions for our accreditation paperwork, please complete this form.
You'll be asked to submit:
- Your AHPRA registration certificate
- Your Medical Indemnity Certificate of Currency
- Your QI/CPD Statement from the RACGP for the 2023/2024/2025 triennium
- Yellow Fever Certification from the ACRRM (optional)
- COVID Vaccination Training (optional)
- Indigenous Sensitivity Training in General Practice (optional)
You may need to organise these documents prior to returning to this page!
Please supply copies of these documents in PDF format (each certificate can be downloaded from the respective certification body from their websites using your provider-specific log on).
