Terms of Use:

• This service is to order REGULAR MEDICATIONS or REFERRALS and for conditions previously known to your GP.
• If you've NOT attended the practice in person in the last 6 (SIX) months, please do NOT complete this request. Please book an appointment (in-person preferred) with the next available GP.
• ONE (1) Patient per submission.
• Your regular GP's consulting days are listed in the selection window below. Requests may not be completed outside of these consult days.
• All requests once submitted at FINAL and cannot be redirected to another provider unless your usual GP has a locum arrangment in place.
• Referrals CANNOT be back-dated. It is your responsibility to ensure that your referral is in date before attending a specialist or allied health service. Referrals are valid for TWELVE (12) months from their date of first use.
• This service is NOT COVERED BY MEDICARE, and attracts a $40 out-of-consult fee. As it is a non-Medicare service, centrelink issued pension and Health Care Cards do not apply.
• If your request requires further investigations, you will be refunded and required to make an appointment (FEES APPLY).
• Completion of requests is contingent on your GP's days of operation, weekends and public holidays. PLEASE CONSIDER THIS WHEN SUBMITTING YOUR REQUEST.
