NB: This form should be used to add/remove sessions, shorten/extend finishing or break times, or move patient appointments to new/altered sessions.

N.B: Some appointments may remain visible in the appointment book until contact has been confirmed.

You will receive EMAIL confirmation once your change of routine has been entered and confirmed.

If your change of routine applies to multiple days, please list ALL days/dates in the details of change field!

Any patients already booked will be notified by SMS or PHONE should their existing appointments need to be moved.
