Before you start...
Requests for Medical Certificates can ONLY
be completed if:
- the request is made within THREE (3) days
of the associated consult (with limited exceptions)
- if there was an error or omission in an existing certificate (including a certificate of capacity)
- Certificates for vaccination status or history are available from your My Health Record, but can be produced on request.
Your doctor may need you to attend the practice to complete a certificate, and such a decision is at their discretion.
