Patient Privacy
Any requests for records or information must be made in writing, and signed, or made in person and in consult.
Children over the age of 16 MUST sign any transfer of records requests; their parents CANNOT sign on their behalf.
Unless explicitly instructed, patients can NOT collect the results of their partners.
If it appears that results or information has been distributed without patient consent, an incident report MUST be completed.
