Referral and Rx Request

A patient requests a referral or prescription

1. Direct the patient to request their prescription through the practice's web portal at churchstreetmedical.com.au


2. If they are unable to use the web portal, you can use it FOR them:



  1. Press the TOP MIDDLE (Rx) button on the Stream Deck (see above) to open the Rx Request page from the web portal
  2. Enter the details of the medication or referral, including if the patient wants the script sent to their PHONE or PHARMACY (NB: if sending to PHARMACY/SPECIALIST, make sure you get the specific name - knowing the email address isn't always necessary, but can be helpful)!


Once done, click the PROCEED TO PAYMENT button. Once the page changes, you can close the window.


The admin staff member will then direct the request where needed - you're done!

It is ILLEGAL to back-date referrals of ANY KIND. If a patient requires a referral ON THE DAY, please ask a senior doctor to complete the request, but let the patient know that it MAY NOT BE POSSIBLE!!!
