Using This Service

1. This service is to request medications and referrals for ongoing (chronic) conditions.

2. Your doctor may need you to attend the practice to complete a certificate, and such a decision is at their discretion.

3. Referrals can ONLY be requested IN ADVANCE of a specialist appointment. It is illegal to back-date referrals of ANY KIND.

4. Medications must be reviewed frequently for efficacy and to ensure proper use. If you have not attended CSMP in more than 6 months, you will be asked to make an appointment to review your medications.

5. For all requests, please allow up to 48 hours for completion. Once your prescription or referral is ready, you will receive  SMS  confirmation either in the form of an eScript or a notification that your referral or script has been transmitted. It is your responsibility to locate a pharmacy that is authorised to dispense eScripts should you choose this method of delivery.

6. If your request is urgent (i.e needed within the next 12 hours) , please contact the Practice on

7. If you request to collect your referral/script from the Practice, it will be held on-site for ONE (1) month. If it has NOT been collected within that time, referrals and scripts will be destroyed and a $30 processing fee will apply to re-print.
