"I'd like my records transferred here" OR if we
receive a request for records from another practice.
Every now and again, we'll receive a request for records from another practice. When we receive that request it needs to be allocated to the Practice Manager.
If the request comes from a practice that does NOT appear in our contacts, please ADD THEM.
However, if a patient is COMING to CSMP, you'll need to request their records. Follow the process above if their old practice is NOT in our contacts, and then do the following:
1. Head to Correspondence Out from the patient file in Best Practice, then click ADD:
Once the Word Processor template opens, select "TMR Request" from the favourites in the left hand column:
From here, you'll then need to select the requesting
Doctor:
Followed by the relevant addressee, whom you should have
added earlier if they don't appear in our Best Practice address book (N.B: the form auto-populates with all relevant data):
The next prompt will be for the details of the request (i.e. whether you're after the full record, pathology/imaging, letters of summaries):
Once you click insert on the last prompt, the template will be completed with all relevant detail that you'll need before faxing off the request!
